Nominated Supervisor Consent Form
The above document must be completed by nominated supervisors in order to undertake regulatory supervision and be forwarded to the Council for approval.
The above document provides detailed guidelines about commencing supervision, the activities to occur while being supervised and the reporting requirements to Council.
The above document is the prescribed report template provided to approved supervisors. The Council reviews reports using this template to monitoring the practitioner’s compliance and progress with conditions.
